You will find attached the updated “Pre-boarding health declaration questionnaire”, that all passengers travelling to and from Greece must fill in and hand over upon embarkation to the vessel's Chief Purser along with their ticket.

A paragraph referring to the collection and processing of personal data has been added to the updated questionnaire.

Please note that the questionnaire is no longer common for the ANEK - SUPERFAST JOINT VENTURE. The passenger must hand over upon embarkation the completed questionnaire of the respective company according to departure vessel (questionnaire ANEK or questionnaire ATTICA).
